S.E.A.L.S. WAIVER & RELEASE OF LIABILITY

Organization: Southeast Atlanta League of Softball (“S.E.A.L.S.”)
Effective Date: 2026 Season

I. VOLUNTARY PARTICIPATION & INHERENT RISKS

I hereby acknowledge that I am voluntarily participating in sports activities organized by the Southeast Atlanta League of Softball (hereinafter referred to as "S.E.A.L.S."). I understand that softball is a physical activity involving inherent risks, including, but not limited to, collisions, falling, being struck by equipment, bodily injury, property damage, communicable diseases (including COVID-19), and death. I acknowledge that these risks may occur as a result of my participation, the actions of others, or the condition of the facilities.

II. RELEASE AND WAIVER OF LIABILITY

I hereby release, forever discharge, and hold harmless the S.E.A.L.S. organization, its directors, officers, agents, employees, umpires, sponsors, sponsoring agencies, other participants, the Kirkwood Neighborhood Association, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise.

III. ASSUMPTION OF RISK

I voluntarily assume all risks associated with my participation, including the risk of exposure to or infection by communicable diseases. I understand that such exposure may result in personal injury, illness, permanent disability, and death. This agreement is binding upon me, my spouse, my heirs, my children (including any guardian ad litem), my assignees, and legal representatives.

IV. INDEMNIFICATION

I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my participation in S.E.A.L.S. activities or any damage I may cause to the facilities or third parties.

V. MEDICAL TREATMENT

I authorize S.E.A.L.S. and its agents to obtain or provide medical treatment for myself or my child in the event of injury or illness. I agree to be responsible for all costs associated with such medical care and transportation.

VI. EQUIPMENT & FACILITY INSPECTION

I agree that prior to participating, I will inspect the facilities and equipment. If I believe anything is unsafe, I will immediately advise a league official and will refuse to participate until such condition is corrected. I assume full responsibility for the safety and maintenance of my personal equipment.

VII. MEDIA & PHOTO RELEASE

I hereby grant S.E.A.L.S. permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including social media and web-based publications, without payment or other consideration.

VIII. CODE OF CONDUCT & GOVERNING LAW

I agree to abide by all S.E.A.L.S. rules. I understand the league reserves the right to terminate my participation for conduct detrimental to the league. This agreement shall be governed by the laws of the State of Georgia, and any legal action shall be filed exclusively in the courts of DeKalb or Fulton County, Georgia.

IX. ACKNOWLEDGEMENT OF UNDERSTANDING

By participating in any capacity in S.E.A.L.S., participants acknowledge that they have read and understood this Waiver and Release of Liability and voluntarily agree to its terms. 
I understand that I am giving up substantial rights, including my right to sue the Released Parties for injury, death, or damages.
